Print off this form and take it with you to the assigned location. Make notes while evaluating

the property in order to help when submitting your report. The form is designed so that any 'yes'

answers mean everything looks good and any 'no' answers mean something is not right. If you

record a 'no', please write up why in the comments section, and have a corresponding photo

All reports must be submitted within 12 hours of completing the property inspection. Attach all photos to an

email to the scheduler at juneshopaudits@gmail.com. Photos: We want you to take a photo of the front signage of

the mall to begin with. Then take 2 or 3 photos corresponding to each section on the print report form. Since there are 6

sections, we would like to see about 12 to 18 photos taken. Of course, if you see any problem areas such as excessive weeds

or litter, or any graffiti on the property, add these photos as extra ones. Send all photos to the scheduler.
